REPRESENTATIVES MAY HAVE TO MAKE A LEGAL CLAIM OR SUE IF YOU ARE INJURED OR DIE BECAUSE OF
YOUR PARTICIPATION IN THE BIRTHDAY PARTY EVENT

RELEASE OF LEGAL RIGHTS
In consideration of my acceptance of my entry, I do hereby, for myself, my heirs, executors and
administrators waive, release, and forever discharge any and all rights and claims for damages
which may have or may accrue to me against Champion Tae Kwon Do Academy, Inc., 2003 Tae
Kwon Do Birthday Party event, all members of the Birthday Party, or their respective officers,
agents, representatives, successors, and/or assigns, and against any Birthday Party guest for any
and all damages which may be sustained to me in connection with my association with or entry in
the 2003 Tae Kwon Do Birthday Party event, or which may arise out of traveling to, participating
in, and returning from this event. I understand that participating in the sport of Tae Kwon Do
involves an element of danger, even when classes are supervised and protective gear is used.
Despite this danger, I want to participate in the Champion Tae Kwon Do Birthday Party 2003, and
I knowingly and voluntarily assume all risks involved in participating in the Birthday Party Event.
I understand the rules of this tournament and agree with them in their entirety. I also understand
that the tournament director has the right to assign, alter and/or change any division for any
competitor(s) during the Birthday Party Event.
